How to REFER to Home Caregivers Home Health

Home Caregivers Home Health is a provider for hundreds of insurance panels. To learn if we can accept your insurance for coverage of services, please call us at 801.485.6166 and ask for our Benefits Coordinator. She will assist you in determining what your policy will and will not cover. If you need help, and we cannot serve you due to insurance restrictions, we will be happy to assist you in finding a reputable substitute.

If you are a hospital, doctor’s office or discharge planner and wish to refer a patient to us for service, please call 801.485.6166 24/7. If after hours, press one and you will be connected with a nurse who can accept the patient for admission to services. Typically, we can admit a patient within 24 hours or less of your call. To fax orders, face sheets or other information please dial 801.531.1949. Since we may not be in the office after hours, call the after-hours nurse to let her know you have faxed information to us. We are nearly always hiring for the following positions:

  • Personal Care Aide: Aides provide personal care to clients as well as assistance with daily living tasks, including help with bathing, showering and dressing, incontinence care, meal preparation, transportation, grocery shopping and running errands, light housekeeping, medication reminders, and other tasks as outlined in the Plan of Care. No experience necessary – we will gladly train you.
  • Certified Nurse Aide: C.N.A.’s provide skilled clinical care for our patients including bowel care, transferring, shower, and bathing assistance, medication reminders, incontinent care, blood pressure, blood sugar testing and a myriad of other skilled services. C.N.A.’s must have a valid C.N.A. certificate, current TB test, and First Aid and CPR certification. They must have reliable transportation and auto insurance, a clean driving record, and must be able to pass a thorough background check and reference verification.
  • Registered Nurse and Licensed Practical Nurse: Our nurses provide skilled care to adult and pediatric patients. We also provide school nursing. Nurses must have a current and valid nursing license, current T.B. test, and first aid and CPR certification. Nurses must have reliable transportation and auto insurance, a clean driving record, and must be able to pass a thorough background check and reference verification.
